Calf pain after knee surgery: causes and warning signs

Tell the surgical team about pain that is new, increasing, unexplained, or accompanied by one-sided swelling, warmth, redness, or tenderness.

After an arthroscopy, ligament procedure, meniscus operation, or other knee surgery, the calf may compensate for a changed walking pattern. Reduced movement can produce stiffness, and swelling or bruising around the knee can extend lower into the leg. A muscle strain may cause pain with pushing through the foot or moving the ankle.

Deep vein thrombosis is a different possibility after surgery and immobility. It may present with calf pain, swelling, warmth, colour change, or tenderness, but it cannot be confirmed or excluded by how the leg feels. Medical assessment may include an examination and an ultrasound.

Use the postoperative plan supplied for walking, exercises, elevation, wound care, and medicines. Compare the affected calf with the other side and record changes in swelling, skin colour, warmth, and pain. If the symptom is unexplained or worsening, call the surgeon's office or the designated after-hours service without waiting for the next routine appointment.

Do not knead the calf, force a stretch, or increase exercise when a clot is a concern. Take any prescribed anticoagulant exactly as directed and seek advice rather than skipping or doubling a dose.

Call emergency services for sudden breathlessness, chest pain, coughing blood, collapse, or faintness. Arrange urgent assessment for substantial or increasing swelling in one calf, redness, unusual warmth, severe tenderness, new foot weakness or numbness, a cool discoloured foot, fever, or spreading redness around the surgical wound. These warning signs should guide urgency regardless of how common a harmless cause might be.

Describe the exact operation and date, when the calf pain started, what makes it worse, and whether swelling or skin changes are present. Ask if you need same-day assessment, whether clot testing is appropriate, and how activity, compression, and medication should be adjusted until you are reviewed.
