Calcaneus stress fracture X-ray: what it can show

Short answer
An X-ray can detect some calcaneus stress fractures, but an early image may appear normal even when the heel bone is injured.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Early X-ray
- May not display a developing stress fracture
- Interpretation
- Combined with symptoms and examination
- Until reviewed
- Avoid impact that reproduces heel pain
The short answer
The result must be interpreted alongside the location of tenderness, pain during loading, recent activity changes, and the physical examination. A normal report does not by itself settle the cause of ongoing focal heel pain.
How to understand the image
Stress injury begins when repeated load outpaces the heel bone's ability to repair itself. The earliest changes can be too subtle for a plain X-ray. As the bone responds, an image may later show a thin line, an area of increased density, or new bone associated with healing, but those findings require professional interpretation.
If suspicion remains after a normal X-ray, the next step is based on symptom severity and the examination. A clinician may protect the foot and monitor it, repeat imaging later, or request an imaging method that can reveal bone stress more clearly.
What to do while awaiting a diagnosis
Pause running, court sports, jumping, and other activities that provoke heel pain. Do not use a normal early X-ray as permission to continue impact training. Reduce painful walking, choose stable footwear, and follow instructions about a walking boot or crutches if these are prescribed.
Keep track of where the pain sits, whether squeezing the heel triggers it, and whether swelling or limping develops. These details help at follow-up. Pain relief should be discussed with a clinician or pharmacist if you have other medical conditions, take regular medicines, or are unsure what is suitable.
When reassessment is needed
Arrange follow-up if heel pain remains focal despite reduced activity, worsens during ordinary walking, or repeatedly returns with exercise. Mention prior stress fractures, menstrual or hormonal changes, restrictive eating, recent training increases, and conditions or medicines that may affect bone health.
Get urgent help if the foot becomes numb, pale, cold, markedly swollen, or visibly deformed, or if you cannot take steps after an injury. Fever, spreading redness, or an open wound near the painful area also needs prompt assessment because these findings are not explained by a straightforward stress fracture.
Useful questions at the appointment
Ask how strongly the examination suggests a calcaneus stress injury, whether another diagnosis could explain the pain, and whether further imaging would change treatment. Clarify weight-bearing limits, the purpose of any support device, the expected follow-up, and the functional signs required before resuming running.

Questions people ask
Does a normal X-ray rule out a calcaneus stress fracture?
No. Early bone stress may not yet create a change visible on a plain X-ray, so persistent focal heel pain deserves clinical reassessment.
Why might another scan be requested?
Different imaging methods reveal different features of bone injury. A clinician may request another scan when the examination remains concerning or when the result would alter protection and return-to-activity advice.
Should I keep running until the diagnosis is confirmed?
Avoid running and other impact that triggers the pain. Continuing to load a suspected stress injury can make symptoms worse while you await a clear plan.
