What should you know about bursitis hip swelling?

Short answer
Hip bursitis can cause local tenderness and fullness, but obvious hip swelling may have another cause and deserves assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Appearance
- Hip bursitis can be painful without producing an obvious surface swelling
- Do not do
- Do not puncture, squeeze, or attempt to drain the area at home
- Useful observations
- Track changes in size, warmth, redness, bruising, drainage, and weight-bearing
What swelling around the hip can mean
A bursa is a thin, fluid-containing cushion between tissues that move against one another. When a bursa near the outer hip is irritated, pain with pressure or side-lying is often clearer than visible swelling because the structure lies beneath thicker tissue. A noticeable lump, broad puffiness, bruising, or swelling extending into the thigh can come from other sources, including a soft-tissue injury, a collection of fluid, or inflammation in nearby structures. A clinician may compare both sides and examine the skin, hip movement, tenderness, strength, and circulation before deciding whether imaging or another test is useful.
How to respond safely
Note when the swelling appeared, whether it is changing, and whether a fall, injection, new exercise, or prolonged pressure preceded it. Reduce direct compression and movements that clearly worsen pain, but keep comfortable movement in the hip and knee unless an injury makes weight-bearing difficult. A cloth-wrapped cold pack can be used for short periods for soreness; do not place ice directly on the skin. Avoid pressing, puncturing, or trying to drain a swollen area yourself. If medicine is being considered, a pharmacist or clinician can check for interactions and health conditions that make anti-inflammatory treatment unsuitable.
Signs that need prompt care
Get same-day medical advice for swelling that is hot, red, rapidly enlarging, draining fluid, or accompanied by fever or feeling markedly unwell. Urgent evaluation is also appropriate after a significant injury, when the leg cannot take weight, or when severe pain is escalating. Call emergency services for sudden swelling of the leg with chest pain, faintness, or difficulty breathing. New swelling without these warning signs should still be examined if it persists, grows, becomes firm, or interferes with walking, because treatment depends on identifying what is swollen rather than assuming the bursa is responsible.
What to ask at an appointment
You could ask whether the swelling appears to involve a bursa, skin, muscle, tendon, or the hip joint; whether infection or injury has been excluded; and what changes should trigger reassessment. If aspiration is proposed, ask what fluid will be tested for and how the result could alter treatment. If rehabilitation is recommended, clarify which positions compress the sore area and how activity should be increased.

Questions people ask
Is a swollen outer hip always bursitis?
No. Bursitis can present with local pain and tenderness, but visible swelling also occurs with other soft-tissue, skin, fluid, or injury-related problems. Examination helps distinguish them.
Should a hip bursa be drained?
Not routinely. A clinician may consider aspiration when identifying the fluid or relieving a suitable collection would guide care. The procedure should be performed under appropriate clinical conditions, not at home.
Can I massage swelling over the hip?
Avoid forceful massage over a new, hot, bruised, or unexplained swelling. Gentle treatment around the area may be appropriate only after the likely cause and safe level of pressure are clear.
