Brown Spotting While Pregnant: What It Can Mean

Spotting means a small amount of blood, often seen on toilet paper or underwear. Brown blood has had time to darken as it leaves the body. It can occur after sex, a vaginal examination, or irritation of the cervix, which becomes more sensitive during pregnancy. It can also occur without an obvious trigger.

The timing and pattern matter. Light spotting early in pregnancy may settle, but bleeding can also occur with a threatened miscarriage or an ectopic pregnancy. An ectopic pregnancy develops outside the womb and needs prompt assessment, particularly when bleeding is accompanied by one-sided pelvic pain, shoulder-tip pain, dizziness, or fainting.

Later in pregnancy, blood-stained mucus can occur as the cervix changes before labour. Bleeding at any stage can have other causes, including infection or changes on the cervix. A symptom does not establish a diagnosis: a clinician may need to ask about the amount and colour of blood, examine you, and arrange an ultrasound or other tests.

Contact your obstetric, midwifery, or maternity service and describe when the spotting began, how much there is, its colour, and whether it is continuing. Mention your stage of pregnancy, any recent sex or examination, your blood group if known, and symptoms such as cramps, fever, unusual discharge, or pain.

Use a pad rather than a tampon so you can observe the amount. Avoid sex until you have received advice if it brings on bleeding or pain. Do not insert anything into the vagina or take medication for bleeding unless a qualified clinician has advised you. Keep any appointment even if the spotting stops, because the episode still helps your maternity team assess what happened.

Seek urgent medical help now for heavy or increasing bleeding, severe or persistent abdominal or pelvic pain, pain mainly on one side, fainting, marked dizziness, shoulder-tip pain, fever, or feeling very unwell. In later pregnancy, contact maternity services urgently for bleeding with regular contractions, leaking fluid, or reduced fetal movement.

If you cannot reach your maternity service, use local emergency services. Take your pregnancy records if this is practical, but do not delay for paperwork. Mild spotting without these warning signs still merits prompt advice, especially if it recurs or follows pain.

You could ask: What might explain this spotting at my stage of pregnancy? Do I need an examination, ultrasound, or blood tests? Which changes should prompt emergency care? Should I avoid sex, exercise, or any medicines while I am being assessed? If my blood group is known to be Rh-negative, do I need specific advice?
