Broken pedicle screw: what it means and what happens next

The decision depends on symptoms, spinal alignment, whether the intended fusion has become solid, and whether any implant has moved. Contact the spine team that performed or follows the surgery and avoid changing activity solely from an imaging report unless they advise it.

Pedicle screws connect vertebrae to rods so the operated segment stays supported while bone fusion develops. A screw may fatigue when repeated load continues across the metal, particularly if the bones have not joined firmly. Implant position, bone quality, spinal balance and a new injury can also affect the construct.

Some breaks are found on follow-up imaging without new symptoms. In other cases, pain or a change in alignment leads to investigation. The image must be interpreted alongside the examination and earlier scans; the broken screw alone does not show whether nerves are affected or the fusion is stable.

Arrange review with a spine surgeon and take previous scan reports if available. The clinician will ask about pain, function and any new nerve symptoms, then compare current images with earlier ones. X-rays may assess alignment and movement; other imaging can examine fusion, implant position or pressure around nerves.

Observation may be appropriate when the spine is stable and symptoms are manageable. Persistent pain, loss of correction, an unhealed fusion, loose or displaced hardware, or nerve compression may lead to discussion of revision surgery. The plan can include removing, replacing or extending hardware and addressing an unhealed fusion.

Seek emergency assessment for new loss of bladder or bowel control, numbness around the groin or saddle area, or rapidly worsening weakness in an arm or leg. Sudden severe back pain after a fall, inability to stand safely, or new breathing difficulty also warrants urgent help.

Contact the surgical team promptly for increasing wound redness, drainage, fever with worsening spinal pain, new tingling or numbness, or a clear decline in walking or balance. These symptoms require clinical assessment rather than waiting for routine imaging follow-up.

Ask whether the fusion appears solid, whether the screw or connected rod has moved, and how current images compare with earlier ones. Clarify which activities are safe now, whether further imaging is planned, what improvement revision surgery could reasonably offer, and what risks follow from monitoring instead.
