When stronger support may be used

The big toe is important for balance and push-off during walking. Treatment is chosen after checking fracture position, the joint surface, skin and nail, and whether the toe remains stable. Some injuries can use a stiff-soled shoe or boot instead of plaster.

Why treatment differs between toe fractures

A fracture that is displaced, enters a joint, remains unstable or has an associated wound needs closer orthopaedic assessment. Realignment may be performed before support is applied. Surgery can be considered when a satisfactory position cannot be maintained by simpler measures.

A cast may extend around the foot or lower leg rather than enclosing only the toe. This limits bending and protects the forefoot, but it also means you must follow specific instructions about weight-bearing and walking aids.

Protect the toe during recovery

Use the shoe, boot, cast and walking aid exactly as fitted. Rest from movements that increase toe pain, elevate the foot when swollen, and keep any plaster and padding dry. Do not test healing by repeatedly bending the toe or walking farther than advised.

Inspect exposed toes for colour, warmth and sensation. If the clinician recommends taping, place padding between the toes and follow their method; tape that is too tight can irritate skin or affect circulation. Keep scheduled review appointments so alignment and joint movement can be reassessed.

When to arrange urgent review

Seek prompt assessment if the toe appears newly crooked, pain or swelling is increasing, the nail is lifting over a blood collection, a wound is present, or the cast becomes wet, damaged or painfully tight. Redness spreading from broken skin, discharge or fever may indicate infection.

Get urgent help when the toe becomes blue, pale, cold, numb or difficult to move, or when severe pressure persists after elevation. People with reduced foot sensation or circulation should report skin changes early because rubbing may be harder to detect.

Ask whether the fracture affects the big-toe joint, whether it has moved out of position, and why the recommended support suits its stability. Clarify how much weight you may place through the foot, when support can change, and which footwear will be appropriate afterward.

Can I walk in a cast for a broken big toe?
