Swelling can be mild, delayed or difficult to see. Its absence cannot confirm that the bones and supporting tissues are uninjured.

Why swelling may be absent

Ankle fractures vary in location and stability. A small or well-aligned fracture may produce less visible tissue reaction than a displaced injury, and swelling may not develop immediately. Ice, elevation or compression used soon after injury can also reduce its appearance.

Clues that still warrant attention include focal tenderness over a bone, pain when standing or walking, bruising, reduced movement, or pain that persists rather than settling. An examination and, when indicated, imaging distinguish a fracture from a sprain or other injury.

Protect the ankle until it is assessed

Stop activities that increase pain and avoid testing the ankle repeatedly by walking on it. Keep it supported in a comfortable neutral position, elevate it, and apply a wrapped cold pack for short intervals if that feels helpful. Do not place ice directly on the skin.

Arrange clinical assessment after a significant twist, fall or impact when there is bony tenderness or difficulty bearing weight. Follow the clinician's instructions about crutches, a boot or a cast; the correct support depends on the fracture pattern and whether the joint remains aligned.

When care is urgent

Seek urgent care for an ankle that looks deformed, an open wound near the injury, uncontrolled pain, or inability to move safely after trauma. Immediate assessment is also needed if the foot becomes cold, pale or blue, or if numbness or worsening tingling develops.

If a support or bandage feels increasingly tight, elevate the limb and obtain prompt medical advice rather than removing a prescribed cast yourself. New calf pain, marked calf swelling, chest pain or breathlessness requires urgent evaluation.

Can I walk on an ankle if it is not swollen?

Lack of swelling does not show that walking is safe. If weight-bearing is painful or there is focal bony tenderness after trauma, protect the ankle and seek assessment before continuing normal activity.

Will a fracture always bruise?

No. Bruising may be absent, subtle or delayed. A clinician considers the injury mechanism, exact site of tenderness, movement and ability to bear weight rather than relying on bruising alone.
