How plaster supports the fracture

A plaster cast limits ankle motion so fractured bone ends can remain in the intended position. Early swelling may change how the cast fits, which is why a temporary splint or later cast adjustment can be part of care. A cast that becomes loose may no longer control movement; one that becomes too tight may press on skin, nerves, or circulation.

Pain and swelling are expected after an ankle fracture, especially when the foot hangs down, but they should respond to the advised measures. Follow-up X-rays may be used to confirm that alignment remains acceptable even when symptoms are improving.

Looking after the cast day to day

Raise the ankle above heart level when resting during the early swollen phase, and move the toes gently if permitted. Keep the cast clean and dry, protect it during washing, and never insert objects, powder, or creams beneath it. Do not trim, reshape, or remove plaster yourself.

Use crutches or another aid exactly as taught, and do not stand on the cast unless the fracture team has allowed it and provided suitable protection. Continue safe movement at the knee, hip, and unaffected limbs to reduce stiffness. Take prescribed or advised pain relief as directed and bring any medication questions to a pharmacist or clinician.

Keep scheduled appointments for cast checks and imaging. When plaster is removed, the ankle may feel stiff and the calf may look smaller. A staged programme of movement, strengthening, balance work, and walking progression may be recommended rather than an immediate return to demanding activity.

When the cast needs urgent review

Seek urgent care if toes become blue, pale, cold, increasingly swollen, numb, or difficult to move. Escalating pain, burning, tight pressure, or pain on moving the toes can indicate excessive pressure and should not be ignored.

Contact the fracture service promptly if the plaster cracks, softens, becomes soaked, develops sharp edges, feels very loose, or produces a foul smell or discharge. Fever, spreading redness above the cast, a new injury, chest pain, sudden breathlessness, or painful calf swelling also require timely medical assessment.

Is swelling inside an ankle cast normal?

Some swelling can occur after the injury, particularly with the foot lowered. Increasing pressure, worsening pain, numbness, colour change, cold toes, or reduced toe movement needs urgent review.

Can I walk on plaster if pain allows?

Only if the treating team has explicitly allowed weight bearing. Pain tolerance does not show whether the fracture is stable under load.

What should I do if the cast gets wet?

Contact the fracture service for advice because wet plaster can soften and damp padding can damage the skin. Do not use strong heat or attempt to remove the cast yourself.
