Bright red period blood with heavy flow

Menstrual blood may change from bright red to dark red or brown as the flow slows and the blood is exposed to oxygen. Those shifts can occur within the same period. A flow deserves attention when it causes flooding, repeated leakage, unusually frequent product changes, disrupted sleep, or restriction of normal activities.

Adenomyosis can present with heavy, painful periods, while uterine fibroids and endometrial polyps can present with heavy or irregular bleeding. Perimenopause may change the timing and strength of flow. Irregular ovulation associated with polycystic ovary syndrome can allow the womb lining to build up before it sheds.

Endometrial hyperplasia can present with heavy, prolonged, or unexpected bleeding, so unusual patterns should be investigated. Whatever the cause, sustained menstrual loss can deplete iron and lead to iron deficiency anaemia. The colour of the blood cannot separate hormonal, structural, medication-related, or pregnancy-related explanations.

Record how often you change pads, tampons, cups, or period underwear; whether you combine products; whether bleeding reaches clothes or bedding; and whether clots, pain, or exhaustion occur. A simple diary is more informative than trying to estimate blood volume.

Arrange an appointment when heavy flow is new, persists, worsens, or interferes with daily life. Tell the clinician about cycle timing, pregnancy possibility, contraception, medicines, pelvic pain, pressure, bleeding after sex, and bleeding between periods. They may suggest blood tests, pregnancy testing, an examination, or ultrasound according to the pattern.

Obtain urgent care when the flow rapidly overwhelms period protection or comes with faintness, confusion, severe weakness, breathlessness, chest discomfort, a fast or pounding heartbeat, or intense pelvic pain. Sit or lie down if dizzy, ask someone to stay with you, and avoid driving.

Emergency assessment is needed if pregnancy may be possible and bleeding accompanies one-sided abdominal pain, shoulder-tip pain, marked dizziness, or collapse. Arrange prompt evaluation for bleeding after menopause or heavy flow combined with bleeding between periods, after sex, or with pain during urination, bowel movements, or sex.

Ask whether your symptoms warrant a blood count and iron studies, what an ultrasound could show, and whether the womb lining needs assessment. Discuss treatments in relation to pain relief, contraception, plans for pregnancy, other medicines, and any reason a particular option may not be suitable for you.
