What should you know about breath hold radiotherapy?

During planning, your team may use imaging to see how your body changes as you breathe. You then practise taking a particular breath and holding it for the period needed for a treatment beam. A monitoring system checks your breathing pattern, and the beam is delivered only when your position is within the planned range.

The pause may help move the heart or another nearby organ away from the radiation field, depending on the treatment area and your anatomy. It can also reduce the effect of breathing movement on the target. Breath hold is an added technique, not a different type of radiation; your radiation oncologist will decide whether it is suitable for your plan.

Wear comfortable clothing and follow the instructions about food, drink, medicines, and bladder or bowel preparation if your centre gives them. At the appointment, you will lie in the treatment position and may have an immobilisation device to help you stay aligned. Tell the team before treatment if you have asthma, breathing difficulty, anxiety about holding your breath, or trouble lying flat.

Practise the breathing pattern exactly as taught. You can breathe normally between holds, and staff can pause the process if you need to release your breath. The treatment team remains outside the room during beam delivery but can see and hear you and can stop the machine when needed.

Contact your radiotherapy team promptly if you develop new or worsening breathlessness, chest pain, faintness, wheezing, or difficulty recovering your normal breathing after a hold. Tell them if pain, nausea, weakness, or difficulty lying still is making treatment harder. Seek urgent medical help for severe breathing difficulty, sudden chest pain, or a new fainting episode.

Ask which organ or movement the technique is intended to protect, how long each breath hold is expected to last, and what happens if you cannot complete a hold. You can also ask whether the technique changes your appointment routine, positioning, or treatment plan, and whom to contact if breathing symptoms appear between sessions.
