Breast MRI with contrast: what to expect

It can provide information that complements, but does not automatically replace, mammography or ultrasound. The reason for the scan and your individual history determine which imaging tests are appropriate.

The contrast agent is given through a small intravenous line, usually in the arm. Images taken before and after injection allow the radiologist to compare how areas of the breast absorb and release contrast. An enhancing area is not a diagnosis by itself; its shape, distribution and timing are interpreted alongside other images and your clinical details.

Contrast-enhanced breast MRI may be requested to clarify an imaging finding, map the extent of a known breast cancer, assess certain symptoms or support screening for someone with a higher-risk history. A radiologist may recommend targeted ultrasound, mammography, follow-up imaging or biopsy if the MRI needs clarification.

Tell the imaging team if you might be pregnant, are breastfeeding, have kidney disease, have reacted to MRI contrast before, or have an implant, device or metal fragment. Bring prior breast images if the centre cannot access them. Follow the centre's instructions about food, medicines and timing; these can vary according to your health and the planned examination.

During the scan, you lie face down with each breast positioned in a cushioned opening. The table moves into the scanner, which makes loud knocking sounds. You will need to remain still and can communicate with the technologist. Mention claustrophobia or difficulty lying prone beforehand so the team can discuss practical support.

Tell staff immediately during or after injection if you develop breathing difficulty, swelling of the face or throat, widespread hives, severe dizziness or feel faint. Emergency care is needed for severe breathing trouble or rapidly increasing swelling.

Mild discomfort or bruising can occur around the intravenous site. Contact the imaging centre promptly if pain, redness or swelling at that site is worsening rather than settling. Do not delay assessment of a new breast lump, bloody nipple discharge, skin dimpling or marked breast redness while waiting for an MRI appointment or result.

Ask what clinical question the MRI is intended to answer, whether mammography or ultrasound is also needed, how your kidney function and previous contrast reactions affect planning, and when the written report should be available. If an abnormality is found, ask which finding needs follow-up and whether comparison with earlier images could change the recommendation.
