Breast Cancer Symptoms Without a Lump

Possible changes include a new difference in breast size or shape, skin dimpling or puckering, redness or thickening, swelling in part or all of the breast, or an area that looks unlike the other side. The nipple may turn inward, change direction, become scaly, or develop discharge that is unexpected, particularly if it is bloody. Swelling or a change in the skin or nipple can be present even when you cannot feel a mass.

These findings can also have non-cancerous causes, such as infection, inflammation, injury, hormonal change, or a benign breast condition. A symptom alone cannot identify its cause. A clinician considers its timing, examination findings, personal history, and any imaging results.

Arrange an appointment for a new change that does not settle, even if you cannot feel a lump. Note when it began, whether it is on one side, whether it changes with your menstrual cycle, and any nipple discharge, pain, fever, or recent injury. Do not squeeze the nipple to test for discharge.

The clinician may examine the breast, nipple, skin, and nearby lymph nodes. Depending on your age, symptoms, and examination, they may recommend a mammogram or breast MRI. If imaging shows an area that needs clarification, a breast biopsy can examine tissue directly. Imaging is used to investigate a concern; it does not by itself establish the diagnosis.

Contact a healthcare professional promptly for a new persistent breast or nipple change, bloody or spontaneous nipple discharge, skin dimpling, or one-sided swelling. Seek urgent medical help if breast redness, warmth, swelling, and severe pain occur with fever or you feel acutely unwell, because an infection may need prompt treatment. If you are pregnant or breastfeeding, mention this when arranging care; breast changes still warrant an appropriate clinical assessment.

You could ask: What could explain this change? Which examination or imaging test is appropriate? When should I expect the result, and what would happen if the imaging is unclear? Should I return if the change continues after an initial reassuring assessment?
