Breast cancer care when you have kidney disease

The oncology and kidney teams may adjust medicines, imaging plans and fluid management according to kidney function and the breast cancer features. Do not stop prescribed cancer or kidney medicines without clinical advice.

Kidneys remove waste, balance salts and fluid, and help clear some medicines. Reduced kidney function can allow certain medicines or their by-products to accumulate. Other breast cancer treatments may need closer blood-test monitoring even when their dose is not primarily determined by the kidneys.

Imaging contrast, dehydration, infection and medicines used for pain or nausea may also affect kidney care. The team weighs these factors against the information a scan or treatment is expected to provide. Kidney disease does not by itself determine which breast cancer treatment a person can receive.

Give the oncology team an up-to-date medicines list, including non-prescription painkillers, supplements and herbal products. Share recent kidney blood-test results, dialysis details if relevant, urine changes and any limits on fluid intake. Make sure the kidney clinician knows about planned scans, surgery and systemic treatment.

Before a contrast scan or new medicine, ask whether kidney tests are needed and whether any regular medicine should be held or timed differently. Follow the fluid advice made specifically for you; drinking extra water can be unsuitable for some people with kidney or heart problems.

Contact the treatment team promptly for much less urine than usual, new swelling, persistent vomiting or diarrhoea, inability to keep fluids down, fever, increasing weakness or confusion. These changes can accompany dehydration, infection, medicine effects or worsening kidney function and may require blood tests.

Seek urgent help for severe breathlessness, chest pain, fainting, sudden confusion, seizures, or rapidly worsening swelling. Use the emergency contact instructions from your cancer centre if symptoms arise during treatment.

Ask how your kidney function changes the proposed breast cancer plan, how often blood and urine checks are needed, and who will coordinate dose decisions. Clarify whether imaging contrast is planned, which pain medicines are suitable, what fluid guidance applies, and whom to call if urine output or swelling changes.
