Breakthrough menstrual bleeding explained

The phrase often refers to bleeding while using hormonal contraception, but similar bleeding without contraception may be called intermenstrual or abnormal uterine bleeding. It describes a pattern rather than a diagnosis.

A changing hormone level can make the uterine lining shed at an unexpected time. This can happen with a new contraceptive method, missed doses, medication interactions, ovulation, or the transition toward menopause. Pregnancy-related bleeding, cervical irritation, infection, thyroid problems, fibroids, and clotting problems can produce a similar pattern.

Some conditions are associated with this presentation: chlamydia can present with bleeding between periods or after sex, and endometrial polyps can present with irregular menstrual bleeding. Neither the timing nor the colour of blood can confirm a cause without considering other symptoms and, when indicated, tests.

Mark the first and last day of each period and every episode between them. Note flow, clots, cramps, bleeding after sex, unusual discharge, contraception use, missed tablets, and other medicines. Check for pregnancy when relevant, because the result changes which causes and treatments need consideration.

Schedule a clinician visit if the change repeats or concerns you. Depending on your circumstances, evaluation may include an examination, pregnancy test, blood count, infection swab or urine test, cervical screening review, or ultrasound. Treatment could involve correcting how a medicine is used or addressing a specific underlying condition.

Seek emergency help for bleeding that is rapidly soaking through period products, fainting, severe dizziness, breathing difficulty, or severe pelvic or abdominal pain. Bleeding after a missed period or positive pregnancy test needs urgent assessment when accompanied by pelvic pain, shoulder-tip pain, weakness, or collapse. Arrange medical review for any postmenopausal bleeding.

Ask whether the pattern points to the cervix, uterus, pregnancy, infection, or a medicine effect; which tests would separate those causes; and whether your contraceptive protection has changed. Before leaving, establish how long to track symptoms, how you will receive results, and what level of pain, flow, or dizziness warrants urgent reassessment.
