Bleeding in early pregnancy: what to do

The term breakthrough bleeding usually describes unscheduled bleeding associated with hormonal contraception. During pregnancy, spotting or bleeding may arise from the cervix, changes around implantation, pregnancy loss, or a pregnancy developing outside the uterus, so the clinical context matters more than the label.

Light spotting can occur after sex because the cervix is more prone to bleeding in pregnancy. Infection can also irritate the cervix; chlamydia may present with bleeding after sex, unusual discharge, or pelvic discomfort, although it can cause no noticeable symptoms.

Appearance cannot establish whether a pregnancy is continuing normally. Brown blood may be older and red blood more recent, but amount, progression, pain, pregnancy dates, examination findings, blood tests, and ultrasound may all guide assessment.

Contact your maternity service, obstetric clinician, or an urgent medical service for individual advice. Tell them the first day of your last menstrual period, any pregnancy-test results, the bleeding colour and amount, whether clots or tissue passed, and whether pain, dizziness, fever, or shoulder-tip discomfort is present.

Use a pad rather than a tampon or menstrual cup while monitoring so the flow is easier to describe. Avoid taking new medicines for pain without checking that they are suitable in pregnancy.

Seek emergency assessment for severe abdominal or pelvic pain, pain focused on one side, shoulder-tip pain, fainting, marked dizziness, confusion, breathlessness, heavy bleeding, or rapidly worsening symptoms. These warning signs can occur with significant internal or external bleeding.

Prompt assessment is also appropriate for fever, offensive-smelling discharge, persistent vomiting, or feeling acutely unwell. Do not wait for bleeding to turn bright red before seeking help when a red-flag symptom is already present.

Ask whether your pregnancy location has been confirmed, what follow-up testing may be needed, and where to go if pain or flow increases. Mention your blood group if known, previous ectopic pregnancy or miscarriage, fertility treatment, contraception around conception, and infection concerns.
