Dark brown breakthrough bleeding: what it can mean

Blood darkens as it remains in the uterus or vagina before leaving the body. A small brown discharge can therefore appear at the beginning or end of bleeding and may occur between expected periods while using hormonal contraception.

Colour alone cannot identify the cause. Hormonal shifts, a late or missed contraceptive dose, a recently started contraceptive method, or irritation after sex can alter the timing of bleeding. Endometrial polyps can present with bleeding between periods, and chlamydia can present with bleeding after sex or between periods, sometimes with discharge or pelvic discomfort.

Consider whether pregnancy is possible, because bleeding in pregnancy needs a different assessment. Note any unusual smell, itching, pain, fever, or change from light staining to a heavier flow.

Record when the bleeding began, its colour and flow, contraceptive doses, recent sex, and accompanying symptoms. Continue prescribed contraception as directed unless a clinician tells you to change it; stopping and restarting hormones can make irregular bleeding harder to interpret.

Take a home pregnancy test if pregnancy is possible, following the kit instructions. Arrange a routine appointment if the staining keeps returning, follows sex, or is accompanied by discharge, because an examination or infection testing may be appropriate.

Seek urgent care for bleeding with severe or one-sided pelvic pain, shoulder-tip pain, fainting, marked dizziness, breathlessness, fever, or feeling very unwell. These symptoms warrant prompt assessment whether the blood looks brown, red, light, or heavy.

Get prompt advice if you are pregnant or could be pregnant, if bleeding is becoming heavier, or if you feel weak. After menopause, any vaginal bleeding should be evaluated rather than assumed to be old menstrual blood.

Ask whether your contraceptive schedule or medicines could explain the pattern, whether pregnancy or infection testing is suitable, and whether the cervix or uterus needs examination. Also ask what change in flow, pain, or duration should trigger a faster review.
