What men should know about a BRCA1 gene change

BRCA1 helps cells repair damaged DNA. A pathogenic or likely pathogenic inherited variant can reduce this repair function and increase susceptibility to cancer, including male breast cancer; a clinician may also consider the wider family pattern when discussing prostate or pancreatic cancer risk.

An inherited variant is present from birth and can pass through either the mother's or father's side. A variant of uncertain significance is different: it should not be treated as a confirmed harmful variant or used by itself to justify preventive surgery.

Obtain the complete laboratory report and arrange genetic counselling. The counsellor can check the exact gene variant, whether it was found in blood or saliva rather than only in a tumour, and whether confirmation is needed.

Bring a family history covering breast, ovarian, pancreatic and prostate cancers on both sides of the family. Your care team can then personalise clinical examinations and screening rather than applying a plan designed for someone else. Share the confirmed result with adult blood relatives through a clear family letter if you feel ready.

Arrange a medical assessment for a new breast lump, nipple discharge, nipple pulling inward, skin dimpling, or swelling in the armpit. Breast cancer in men can present with these changes, although such symptoms can also have non-cancerous causes.

Promptly report blood in the urine, increasing difficulty passing urine, unexplained persistent abdominal or back pain, jaundice, or unplanned weight loss. Urgency comes from the nature or persistence of the symptom, not from assuming that a BRCA1 result has caused it.

Ask whether the report says pathogenic, likely pathogenic, or uncertain; whether the finding is germline; which examinations suit your age and family history; and which relatives should be offered testing for the same variant.
