Braxton Hicks: when should you worry?

Short answer
Worry about Braxton Hicks when tightenings become regular, painful, stronger, or closer together, or when bleeding, fluid loss, pressure, or reduced movement occurs.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What a concerning change looks like
Braxton Hicks are commonly irregular episodes in which the abdomen firms and then relaxes. They may settle after rest, hydration, a position change, or emptying the bladder. Concern rises when the episodes develop a reliable rhythm, build in strength, or continue despite those measures. Period-like cramps, a persistent lower backache, pelvic pressure, bleeding, watery fluid, fever, or a change in fetal movement can signal a need for assessment. The pattern matters more than one isolated tightening.
Steps to take
Stop strenuous activity and write down the timing and strength of each episode. Rest on your side, drink fluids if suitable, and check for associated symptoms without inserting anything into the vagina. Keep your maternity contact details available. If the symptoms continue or you cannot judge the pattern, call your maternity unit and describe exactly what you feel and when it began.
Get medical help promptly
Contact maternity care urgently for regular or painful contractions, vaginal bleeding, fluid leakage, pelvic pressure, persistent back pain, fever, or reduced fetal movement after you have begun noticing movement. Call emergency services for heavy bleeding, severe constant pain, fainting, breathing difficulty, or feeling seriously unwell. Early advice is appropriate when something feels different from your usual pregnancy symptoms.
Questions for your maternity team
Ask what contraction pattern your unit wants reported, how to reach the team overnight, and whether any previous pregnancy history changes your threshold for assessment. Ask how to distinguish fluid leakage from normal discharge.

Questions people ask
Should painful Braxton Hicks be checked?
Yes. Pain that is persistent or increasing should be assessed rather than assumed to be practice tightening.
Is fluid leakage always obvious?
No. A small or ongoing watery leak can be difficult to distinguish from discharge, so contact maternity care for advice.
