Brain metastases treatment: what patients should know

Options may include focused radiation, radiation to the whole brain, surgery, medicines that act throughout the body, steroids to reduce swelling, and medicines to control seizures or other symptoms. A plan can combine approaches or use them in sequence.

Brain metastases are cancer deposits that travelled from another part of the body. MRI findings help the team assess their position and extent. The cancer type and its molecular features can matter because some systemic medicines reach and control deposits in the brain.

Focused radiation can target selected areas while limiting exposure to nearby tissue. Whole-brain radiotherapy treats the brain more broadly. Surgery may be considered when removing an accessible deposit could relieve pressure, provide tissue for diagnosis, or control a dominant tumour.

Bring your current medicine list, previous scan reports and details of the original cancer to appointments. Tell the team about blood thinners, seizures, implanted devices, allergies, pregnancy or possible pregnancy, and any difficulty lying still.

Arrange transport if treatment or medicines may affect alertness. Keep a daily note of headaches, weakness, balance, speech, vision, nausea and seizure activity. Do not stop steroids or anti-seizure medicines suddenly unless the prescribing clinician gives a tapering plan.

Call emergency services for a first seizure, a seizure that continues or repeats without recovery, sudden weakness on one side, new difficulty speaking, collapse, severe confusion, loss of consciousness, or a sudden intense headache.

Contact the oncology team promptly for worsening headache, repeated vomiting, increasing sleepiness, new problems with balance or vision, fever during cancer treatment, or symptoms that return while a steroid dose is being reduced.

Ask what the treatment is intended to achieve, why a local or whole-brain approach fits your scans, whether surgery could help, and how treatment relates to therapy for the original cancer. Clarify expected effects on memory, energy, hair, driving and work, as well as the schedule for follow-up imaging.
