Brain and bone metastases: what they mean and what happens next

A metastasis keeps the type of the cancer where it began. For example, cancer that started in the lung and reached a bone is treated as metastatic lung cancer, not as a new bone cancer. Brain deposits may affect movement, speech, vision, balance, thinking, or seizure control. Bone deposits can weaken a bone, cause local pain, press on nearby nerves, or disturb calcium levels.

The oncology team considers the original cancer, every known site of disease, scan findings, symptoms, general health, and previous treatment. Brain and bone problems may need different local treatments while medicine that travels through the body treats cancer more broadly.

Keep a daily note of headache, nausea, weakness, balance, pain location, mobility, numbness, and any medicines taken. Report new or worsening changes promptly. Take prescribed steroids, anti-seizure medicines, and pain relief exactly as directed; do not stop steroids suddenly unless the treating team provides a taper.

Treatment may include radiotherapy to control a painful or vulnerable bone deposit, treatment directed at a brain deposit, bone-strengthening medicine, systemic cancer therapy, rehabilitation, and symptom support. Before lifting, exercise, or driving, ask whether a weakened bone, seizure risk, or neurological change requires temporary restrictions.

Seek emergency help for a first seizure, loss of consciousness, sudden one-sided weakness, new trouble speaking, severe confusion, or a rapidly worsening headache with repeated vomiting. Do not drive yourself if vision, awareness, strength, or balance is affected.

Urgent assessment is also needed for sudden severe bone pain, inability to bear weight, or a limb that looks shortened or deformed. New back or neck pain with leg weakness, numbness around the buttocks or genitals, or loss of bladder or bowel control may signal pressure on the spinal cord and cannot wait for a routine appointment.

Ask which findings are causing current symptoms, whether any bone is at risk of breaking, whether the spine is involved, and what the brain imaging shows. Clarify the aim of each treatment, how quickly symptom relief may occur, which medicine side effects warrant a call, and whom to contact outside clinic hours.
