What the cramp involves
The brachioradialis runs along the outer forearm and helps bend the elbow, especially when the hand is in a neutral position. Repeated gripping, lifting or unfamiliar exercise can fatigue it and cause a painful knot or twitch. A muscle strain may cause tenderness that lasts beyond the cramp. Dehydration and changes in body salts can also contribute. Less commonly, cramps occur with chronic kidney disease, hypothyroidism, vitamin D deficiency, peripheral arterial disease or reduced muscle mass such as sarcopenia.
How to ease it
Stop the activity and support the arm. Slowly straighten the elbow and gently extend the wrist until the forearm feels stretched, without bouncing or forcing the joint. Light massage and warmth may relax the muscle. Drink fluids if you have been sweating or have had poor intake. Once the pain settles, return to activity gradually and review gripping technique, breaks and warm-up. Do not take supplements or repeated pain medicines for recurrent cramps without checking the cause.
When to seek care
Arrange medical review if cramps keep returning, last unusually long, disturb sleep, follow a new medicine or come with weakness, numbness, swelling or loss of movement. Seek urgent help for a cold or pale hand, severe swelling, sudden weakness or pain after a major injury. A clinician may examine the arm and consider blood tests when an underlying condition or medicine effect is possible.
