What should you know about bowel cancer spread to bones?

This is metastatic bowel cancer, not primary bone cancer. Care can address the cancer throughout the body while also protecting affected bones and relieving symptoms. Decisions depend on the sites involved, previous treatment, test results, and the person's health and priorities.

Bone metastases can disrupt normal bone renewal, weaken an affected area, and irritate nearby nerves. Bowel cancer with bone involvement may present with persistent, localised pain, pain at night, reduced mobility, or a fracture after minor strain, although symptoms differ between people.

Imaging helps identify suspicious areas and assess bone strength. Blood tests may show changes related to bone, kidney function, or treatment readiness. A biopsy is sometimes needed when the diagnosis is uncertain, particularly if a bone finding could have another explanation.

Report new focal bone pain, altered walking, limb weakness, numbness, or changes in bladder or bowel control without waiting for a routine visit. Avoid heavy loading of a painful limb until the team has assessed whether the bone is weakened.

Treatment may include cancer medicines that work throughout the body, radiotherapy to a painful or threatening site, pain relief, medicines that reduce further bone damage, or surgery to stabilise a vulnerable bone. Rehabilitation and mobility aids can help preserve safe movement.

Tumour testing can sometimes identify features that guide medicine selection. The team may also review calcium, vitamin intake, dental health, kidney function, and the need for support with daily activities.

New severe back or neck pain with leg or arm weakness, numbness, difficulty walking, or loss of bladder or bowel control may signal pressure on the spinal cord. Treat this as an emergency and contact the cancer service or emergency services immediately.

Sudden intense pain, a limb that looks deformed, or inability to bear weight can indicate a fracture and also needs urgent assessment. Marked thirst, repeated vomiting, unusual drowsiness, or confusion warrants prompt care because disturbed blood calcium can affect the brain, kidneys, and heart.

Ask which bones are affected, whether any area is at risk of fracture, and which activities are safe. Clarify the aims of systemic treatment and radiotherapy, how pain will be reviewed, and which neurological changes require emergency action.
