The short answer

A report may describe bony projections, bridging bone or syndesmophytes. These terms do not all mean the same thing, and the location and pattern help a clinician decide whether the change fits inflammatory spinal disease or ordinary wear-related change.

How new bone can develop

Ankylosing spondylitis causes inflammation in the joints of the spine and at entheses, where tendons or ligaments attach to bone. Healing after repeated inflammation can lay down extra bone. Along the spine, this may gradually bridge neighbouring vertebrae and reduce flexibility. At the heel or another attachment, the change may resemble a spur.

People with ankylosing spondylitis commonly experience inflammatory back pain and stiffness, often worse after rest or on waking. A spur seen on a scan may cause no symptoms, so pain should not automatically be assigned to that finding.

Ask the clinician who requested the scan where the extra bone is located and whether its shape supports ankylosing spondylitis. Treatment usually targets active inflammation, pain, posture and mobility rather than removing every bony projection.

Keep moving within a comfortable range. A physiotherapist can tailor spinal mobility, strengthening and breathing exercises, while water-based activity may make movement easier during a painful period. Medication decisions should account for your other conditions, current medicines and previous reactions.

When to seek medical care

Arrange an assessment if stiffness is steadily limiting daily movement, pain repeatedly wakes you, or a previously manageable symptom changes. Seek urgent care after a fall or impact if you develop sudden severe neck or back pain, because a stiffened spine can be vulnerable to fracture.

New limb weakness, spreading numbness, loss of bladder or bowel control, or numbness around the groin needs emergency assessment. A painful red eye with light sensitivity or blurred vision also needs prompt evaluation, as eye inflammation can accompany ankylosing spondylitis.

Do bone spurs always need surgery?

Can exercise break or remove a spur?
