Bone scans for stress fractures: a practical guide

How the structures involved in Stress Fracture differ from normal

Short answer

A bone scan can reveal increased activity at a suspected stress fracture, including when an early injury is difficult to see on a plain X-ray.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

What it detects
Bone response to repeated loading
Important caution
A hot spot has more than one possible cause
While waiting
Avoid activity that reproduces focal bone pain

What can the scan show?

A tracer travels through the bloodstream and becomes more concentrated where bone is responding to repeated load. The scan can survey a broad area, but increased uptake is not unique to a stress fracture and may require comparison with targeted imaging.

What an abnormal area means

The shape and precise location of tracer uptake matter. Clinicians compare that pattern with where activity causes pain, whether there is focal tenderness, recent changes in training or work, and any history of injury.

Some stress fractures occur in sites where continued loading can allow the crack to progress. The report therefore helps guide protection of the bone, but the management plan also depends on the bone involved and your examination.

What to do before and after assessment

Pause running, jumping, or any movement that reliably brings on focal bone pain. Do not test the injury repeatedly. A clinician may recommend supportive footwear, a boot, or crutches according to the site and severity.

Before scanning, report pregnancy, possible pregnancy, breastfeeding, recent nuclear imaging, and the exact painful location. Afterward, follow the imaging centre's fluid instructions and obtain a clear plan for activity, follow-up imaging, and gradual return to loading.

Signs that need earlier care

Arrange prompt assessment if pain is worsening, occurs at rest, causes a new limp, or makes it difficult to bear weight. Sudden severe pain or a snap followed by loss of function may signal that the injury has progressed.

Seek urgent care for a visibly deformed limb, a foot that becomes cold or numb, rapidly increasing swelling, or severe pain after a new injury. These features need examination rather than waiting for a routine appointment.

Questions that clarify recovery

Ask which bone is affected, whether you should avoid weight-bearing, what alternative exercise is suitable, and what milestones must be met before returning to sport or work. It is also useful to discuss nutrition, menstrual changes, medicines, and previous stress injuries when relevant.

Questions people ask

Can I exercise while waiting for the result?

Avoid impact and any activity that triggers focal pain. Ask your clinician about low-load alternatives because the safe option depends on which bone may be injured.

Why might another scan be requested?

Another imaging method can define the bone and nearby soft tissues more precisely or help distinguish a stress injury from another cause of increased uptake.

Will pain disappear before the bone is ready for sport?

Symptoms can improve before the bone has regained enough tolerance for full loading. A staged return should use clinical guidance and functional milestones, not pain alone.
