Bone metastases on an X-ray

Doctors interpret the image alongside your symptoms, cancer history and other tests. An X-ray may show an area where bone has been lost, become unusually dense, changed shape or fractured.

Bone metastases are cancer cells that have travelled from another organ and grown in bone; they are named after the original cancer, not as a new bone cancer. Breast, lung and prostate cancers can spread this way.

An abnormal area on an X-ray is not diagnostic by itself. Arthritis, an old injury, infection and benign bone conditions can also alter an image. CT, MRI, a bone scan or PET imaging may be used to define the area, check other bones or assess nearby nerves. Sometimes a biopsy is needed to identify the cause.

Tell the cancer team where the pain is, whether it wakes you, and whether movement or weight-bearing makes it worse. Mention weakness, numbness and changes in bladder or bowel control. The team may arrange further imaging and blood tests, then review the risk of fracture or pressure on the spinal cord.

Care is tailored to the cause and location. Options can include pain medicines, radiotherapy to a painful site, bone-strengthening treatment, systemic cancer therapy or an operation to support an unstable bone. Until fracture risk is clarified, ask whether you should avoid heavy lifting or high-impact activity.

Seek emergency care for new leg weakness, numbness around the buttocks or genitals, trouble walking, or loss of bladder or bowel control. These can signal pressure on the spinal cord and need immediate assessment.

Sudden severe pain, inability to bear weight, or a limb that looks shortened or misshapen may indicate a fracture. Contact your cancer team promptly for new persistent bone pain, rapidly worsening pain or pain that is no longer controlled by the agreed plan.

Ask what the X-ray actually showed, whether another scan is needed, which activities are safe, and whom to contact if pain changes. You can also ask whether treatment is intended to control cancer, protect bone, relieve symptoms, or combine these aims.
