How hip-area infection may feel

Pain may feel deep rather than close to the skin and can worsen when the leg is moved, when standing, or during walking. A person may shorten each step, avoid putting weight through that side, or keep the hip in a position that feels less painful. Children may limp, refuse to walk, or become distressed during dressing and nappy changes.

Possible accompanying features include chills, tiredness, poor appetite, night discomfort, and tenderness over the pelvis or upper thigh. Osteomyelitis can follow bloodstream spread, nearby infection, surgery, or penetrating injury. These symptoms have other possible causes, so diagnosis relies on clinical examination supported by blood tests and imaging.

Preparing for medical assessment

Stop strenuous activity and do not force the hip through painful movement. Note when symptoms began, whether fever occurred, where the pain travels, and whether standing is possible. Tell the clinician about recent infections, wounds, injections, operations, falls, antibiotics, immune-suppressing medicines, and implanted medical devices.

Care may involve blood tests, imaging, cultures, and antibiotics chosen for the suspected or identified organism. A procedure may be needed to drain infected material or remove damaged tissue. Reconstruction, including bone grafting in selected circumstances, is planned only after the infection and structural problem have been carefully assessed.

Red flags requiring urgent care

Go for urgent evaluation if deep hip-region pain occurs with fever, rapidly worsening mobility, inability to stand, or severe pain when the leg is moved. A child who refuses to walk and appears feverish or unusually sleepy also needs immediate medical attention.

Call emergency services for confusion, collapse, breathing difficulty, mottled skin, or profound weakness alongside suspected infection. After a hip operation or injection, new wound drainage, spreading redness, increasing heat, or escalating pain should be reported promptly to the treating service.
