Symptoms of Bone Infection in the Hip

Short answer
Osteomyelitis around the hip can present with deep groin, buttock, or upper-thigh pain, fever, reduced hip movement, and difficulty walking.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

- Urgent combination
- Deep pain with fever or marked illness
The short answer
Symptoms vary with the bone and nearby tissues involved. The hip lies deep beneath muscle, so visible redness or swelling may be absent. New severe pain with fever, inability to bear weight, or marked illness needs urgent evaluation because bone and joint infections require different but prompt treatment.
How hip-area infection may feel
Pain may feel deep rather than close to the skin and can worsen when the leg is moved, when standing, or during walking. A person may shorten each step, avoid putting weight through that side, or keep the hip in a position that feels less painful. Children may limp, refuse to walk, or become distressed during dressing and nappy changes.
Possible accompanying features include chills, tiredness, poor appetite, night discomfort, and tenderness over the pelvis or upper thigh. Osteomyelitis can follow bloodstream spread, nearby infection, surgery, or penetrating injury. These symptoms have other possible causes, so diagnosis relies on clinical examination supported by blood tests and imaging.
Preparing for medical assessment
Stop strenuous activity and do not force the hip through painful movement. Note when symptoms began, whether fever occurred, where the pain travels, and whether standing is possible. Tell the clinician about recent infections, wounds, injections, operations, falls, antibiotics, immune-suppressing medicines, and implanted medical devices.
Care may involve blood tests, imaging, cultures, and antibiotics chosen for the suspected or identified organism. A procedure may be needed to drain infected material or remove damaged tissue. Reconstruction, including bone grafting in selected circumstances, is planned only after the infection and structural problem have been carefully assessed.
Red flags requiring urgent care
Go for urgent evaluation if deep hip-region pain occurs with fever, rapidly worsening mobility, inability to stand, or severe pain when the leg is moved. A child who refuses to walk and appears feverish or unusually sleepy also needs immediate medical attention.
Call emergency services for confusion, collapse, breathing difficulty, mottled skin, or profound weakness alongside suspected infection. After a hip operation or injection, new wound drainage, spreading redness, increasing heat, or escalating pain should be reported promptly to the treating service.
Questions that shape the treatment plan
Ask whether the concern is in bone, the hip joint, or nearby soft tissue; each location can change the testing and procedure plan. Discuss how samples will guide antibiotics, whether weight-bearing should be limited, how medicine effects will be monitored, and which symptoms require immediate return.

Questions people ask
Can hip bone infection cause knee pain?
Hip-region problems can sometimes be felt farther down the thigh or around the knee. The clinician should still examine the hip when knee pain accompanies a limp or limited hip movement.
Does the skin always look red over an infected hip bone?
No. The affected structures can be too deep for obvious skin changes, so deep pain, fever, and loss of movement remain important even when the skin looks normal.
Are osteomyelitis and septic arthritis the same?
No. Osteomyelitis affects bone, while septic arthritis affects a joint. They can produce overlapping symptoms and may occur near each other, so clinicians must identify the involved structures.
