What should you know about bone cement with gentamicin?

A surgeon may select a premixed formulation for a defined orthopaedic situation. The decision considers the operation, infection history or concern, likely organisms when known, allergies, kidney function, and the rest of the medical and surgical plan.

Gentamicin is an aminoglycoside antibiotic. When incorporated into cement, some medicine is released into nearby tissues while the hardened cement performs its mechanical role. The amount and pattern of release depend on the formulation and clinical setting.

Antibiotic cement cannot cover every organism, clean infected tissue, or correct an unstable implant by itself. When clinicians are treating an established joint or bone infection, they may also need samples for laboratory testing, surgical cleaning, implant decisions, and antibiotics delivered by another route.

Tell the surgeon and anaesthesia team about any previous reaction to gentamicin, other aminoglycoside antibiotics, bone cement, or implanted materials. Mention kidney disease, hearing or balance problems, current infections, and every medicine or supplement you take. The team can then assess whether the proposed formulation fits your circumstances.

After surgery, take any separately prescribed antibiotic exactly as directed; the cement is not a reason to skip tablets or intravenous treatment. Attend planned blood tests or follow-up visits if arranged, and protect the wound according to the discharge instructions.

Call emergency services for breathing difficulty, swelling of the face or throat, widespread rash with faintness, chest pain, or collapse. A marked reduction in urination, severe new dizziness, or sudden hearing change after treatment involving gentamicin warrants prompt medical contact.

Increasing wound pain, heat, redness, pus-like drainage, fever with local symptoms, or a new inability to use the operated joint should be reported urgently to the treating service. Antibiotic-containing cement does not rule out an infection.

Ask why gentamicin cement is being considered, whether it is intended as part of prevention or treatment, and what other infection measures are planned. Also clarify whether kidney monitoring is relevant, which allergy history changes the plan, and what wound changes require an immediate call.
