Boils and folliculitis: understanding the difference

Folliculitis may appear after shaving, friction, sweating, or an irritating product. It can look like clusters of pimples, itchy bumps, or spots around hairs on the scalp, beard area, thighs, buttocks, or trunk. When inflammation and infection extend deeper into one follicle, a larger tender boil may develop. Ingrown hairs can create a similar bump, so location, timing, and examination help separate these causes. Many small bumps and one enlarging painful lump are not managed in exactly the same way.

Pause shaving or waxing over affected skin, wear loose clothing, and rinse away sweat after exercise. Use a clean towel and avoid picking or scrubbing the bumps. A clinician may recommend topical antibiotic therapy for selected superficial infections, while deeper or spreading infection may need antibiotics for a skin infection. Do not use leftover antibiotics or share treatment. If a large lump becomes soft in the centre or starts draining, arrange an examination rather than trying to empty it yourself.

Get prompt advice if bumps become increasingly painful, merge into a larger swelling, spread beyond the hair-bearing area, or keep returning after shaving changes. Seek urgent care for fever, rapidly expanding redness, red streaks, severe swelling, or a lesion close to the eye. A persistent itchy scalp eruption or recurrent ingrown hairs also merits a planned review.

Ask whether the pattern is folliculitis, an ingrown hair, or a boil; which hair-removal habits to change; and whether a topical or oral treatment is appropriate.
