The short answer

Hormonal shifts, disrupted sleep, increased physical effort and changes in posture can all contribute. The pattern, location and intensity of pain, along with any other symptoms, help determine whether you need an examination.

Why aches and fatigue can happen

Early in pregnancy, hormonal changes can make you feel unusually sleepy or drained. As pregnancy progresses, the growing uterus shifts posture and places more strain on the back, pelvis, hips and legs. Poor sleep, long periods of standing, dehydration or missed meals may make discomfort and low energy more noticeable.

Fatigue can also occur with anaemia, thyroid problems, infection or another medical condition. Symptoms such as breathlessness, palpitations, fever, painful urination or weakness that limits normal activity give your maternity clinician useful clues and may lead to an examination or blood tests.

What you can do

Pace demanding tasks, take short rest breaks and change position regularly. Gentle movement, supportive footwear, a pillow between the knees at night and comfortable posture may ease muscular aches. Drink fluids regularly and eat balanced meals with sources of protein and iron.

Tell your maternity clinician if tiredness is worsening, does not improve with rest or interferes with daily life. Do not start iron tablets or pain medicines solely on the basis of symptoms; pregnancy-safe choices and the need for iron treatment depend on your health history, current medicines and, when appropriate, test results.

When to seek medical care

Seek urgent medical care for severe or sudden abdominal pain, chest pain, difficulty breathing, fainting, heavy vaginal bleeding, a severe headache with vision changes, or pain and swelling in one calf. Fever with feeling very unwell, fluid leaking from the vagina, regular painful tightenings, or reduced fetal movement after you have started noticing your baby's usual pattern also needs prompt assessment.

Contact your maternity team soon for persistent pain, steadily worsening fatigue, dizziness, a racing heartbeat, painful urination or symptoms that stop you eating, drinking, sleeping or moving normally. If you feel seriously unwell or cannot reach your maternity service, use local emergency care.

Ask whether the location and pattern of your pain fit expected pregnancy changes, whether your fatigue warrants blood tests, which forms of movement are suitable for you, and which pain relief options are compatible with your pregnancy. Bring a list of medicines and supplements, and describe when the symptoms began, what makes them better or worse, and any bleeding, fever, breathlessness or urinary symptoms.

Is it normal to feel tired throughout pregnancy?

Fatigue may occur at any stage and can vary from day to day. Mention it at a maternity appointment if it is persistent, worsening or affecting basic activities, because anaemia and other treatable conditions can cause similar symptoms.

Can I take pain relief for body aches while pregnant?

Some pain medicines are unsuitable during parts of pregnancy or with certain health conditions. Check the medicine, dose and timing with a maternity clinician or pharmacist before taking it.

Should tiredness be treated with iron tablets?
