Bloody sputum from the throat: what to know

Sputum is mucus brought up by coughing. A scratchy throat, nosebleed, gum bleeding, or chronic sinusitis can add blood before mucus is swallowed or spat out. Lower-airway conditions such as acute bronchitis, pneumonia, bronchiectasis, or COPD can also produce blood-streaked sputum. The appearance matters: frothy or bubbly blood with breathlessness is more concerning than a single fine streak after forceful coughing. These conditions can present with bloody sputum; the symptom alone does not establish which one is present.

Spit the mucus into a tissue rather than swallowing it so you can observe its colour and amount. Rest, drink fluids, and avoid smoking or vaping. Record cough, fever, breathlessness, chest pain, and whether the blood returns. Arrange clinical assessment for recurrent blood, a persistent cough, or new breathing symptoms. Antibiotic treatment is not suitable for every cause, so use it only when prescribed for the diagnosis.

Seek emergency care for heavy or ongoing bleeding, severe breathlessness, chest pain, blue lips, fainting, confusion, or sudden deterioration. Prompt review is needed for fever with worsening cough, repeated blood-streaked sputum, or known lung disease. A clinician may listen to your chest, inspect the mouth and nose, and decide whether imaging or other tests are needed.
