Blood Traces in a Cough: What to Know

Forceful coughing can scratch sensitive airway lining, particularly during a common cold or acute bronchitis. Allergic rhinitis may cause nose or throat irritation and blood-streaked mucus, while acid reflux can trigger repeated throat clearing. Asthma, bronchiectasis, bronchiolitis and chronic cough have different mechanisms and need assessment in context. The source may be the mouth, nose, throat or lungs, so note whether blood is mixed with phlegm, follows nasal bleeding, or appears after vomiting.

Note the colour, amount and number of episodes, and whether you have fever, wheeze, chest discomfort or breathlessness. Avoid smoking and vaping, sip fluids if appropriate, and arrange a review when the symptom repeats, persists or occurs with a long-standing cough. Bring a medicine list, including blood thinners, because these can affect bleeding risk.

Get emergency help if you cough up a large amount, continue bleeding, feel faint, struggle to breathe, develop severe chest pain, or become confused or blue around the lips. Seek prompt care for worsening breathlessness, high fever or a person who appears seriously unwell. Keep any blood away from swallowing if possible so its amount and source are easier to describe.
