Understanding swelling under the skin

The location, speed of onset, tenderness, colour and associated symptoms help narrow the possibilities. Sudden swelling of the lips, tongue or throat is an emergency because it may threaten the airway.

A soft indentation that remains briefly after pressing can suggest fluid in the tissues. A painful, warm and increasingly red area can occur with cellulitis. Angioedema often presents with sudden deeper swelling, commonly around the eyes or lips, while hives present with raised, itchy welts that may shift location. A bruise after trauma may combine discolouration with a tender collection of blood.

One-sided limb swelling, a firm defined lump, recent injury, a new medicine, insect exposure and fever each change what a clinician considers. Appearance cannot reliably determine the cause without the wider history and examination.

Remove tight jewellery from a swelling hand or wrist before it becomes difficult to take off. Rest an injured area and apply a wrapped cool pack for short periods, provided there is no open wound or loss of sensation. Avoid forceful massage, puncturing a lump or applying an unfamiliar cream.

Record when the swelling began, possible triggers, medicines taken and whether it changes with position. Mark the edge of visible redness with the time if it is spreading, then seek medical assessment. Recurrent or unexplained swelling also warrants a clinical appointment.

Call emergency services for swelling of the tongue or throat, trouble breathing, wheezing, faintness or rapidly worsening facial swelling. Seek urgent evaluation for a suddenly swollen painful leg, swelling with chest pain, a limb that becomes pale or cold, severe pain after injury, or redness and warmth accompanied by fever. Rapid progression matters even when the initial area looks small.

Ask whether the swelling feels like fluid, inflammation, a blood collection or a defined mass. Discuss whether a medicine or allergy could contribute, whether imaging or blood tests would change management, and exactly which progression signs require same-day reassessment.
