Blood spots under the skin: what they may mean

A flat red, purple or brown mark may come from blood leaking beneath the surface, while a raised tender spot may instead be an inflamed hair follicle or pimple. A clinician can distinguish these patterns by examining whether the marks blanch when pressed, their distribution and any accompanying illness.

Pinpoint flat marks that do not fade under the pressure of a clear glass can represent bleeding into the skin. Larger colour changes may be bruises. By contrast, bumps with a white centre, a visible hair, itch or local tenderness fit an inflammatory skin process more closely. Acne commonly presents with blackheads, whiteheads and inflamed pimples; folliculitis commonly presents with spots centred on hairs.

Recent knocks, vigorous scratching, medicines that affect clotting, a fever, nose or gum bleeding, and similar marks elsewhere are useful context. Colour alone cannot establish the cause, and darker skin may show subtle red tones less clearly.

Note when the marks appeared, whether they are flat or raised, and whether pressing makes them fade. Photograph them in consistent light with a size reference, but do not repeatedly squeeze or scrape them. For a minor bruise after a known knock, protect the area and use a wrapped cool pack briefly for comfort.

Arrange a routine assessment if unexplained marks recur, spread, or accompany easy bruising. Bring a list of prescribed medicines, supplements and recent illnesses; do not stop an anticoagulant or other prescribed treatment without medical advice.

Seek urgent medical assessment for non-blanching red or purple spots with fever, severe headache, neck stiffness, confusion, breathing difficulty or rapid deterioration. Prompt care is also appropriate when widespread marks appear suddenly, bleeding will not stop, there is blood in urine or stool, or bruising follows a significant injury. These features drive urgency regardless of the eventual diagnosis.

Ask whether the marks are bleeding beneath the skin or inflamed follicles, whether any medicine could contribute, and whether an examination or blood test is appropriate. Also ask which changes should trigger urgent reassessment and how to monitor the area safely at home.
