Blood spots on the skin: how to respond

A blood blister is raised and often follows friction or pinching. Flat red, purple, or brownish dots that do not fade under pressure may reflect small amounts of blood beneath the skin. A crusted bump may have bled after scratching, shaving, or squeezing an inflamed follicle.

Distribution matters. One mark at an injury site differs from many new spots across the limbs or trunk. Also check for pain, itch, warmth, pus, bruising, fever, gum bleeding, or a recent medicine change rather than relying on colour alone.

For a minor surface injury, clean the skin and apply firm pressure with clean gauze. Protect an intact blister from rubbing and leave its roof in place. Avoid squeezing pimple-like bumps or applying strong acids, bleach, or unprescribed steroid creams to unexplained spots.

Record when the marks appeared and whether new ones develop. A photo beside a familiar object can show scale without repeatedly touching the skin. Seek a routine appointment for recurrent, unexplained, or persistent spots, especially alongside easy bruising.

Urgent assessment is warranted when rapidly appearing non-fading red or purple spots occur with fever, severe illness, stiff neck, confusion, breathing trouble, or unusual sleepiness. Heavy bleeding that does not stop with pressure, vomiting blood, black stools, or blood in urine also needs immediate care.

Contact a clinician promptly if spots spread quickly, follow a new medicine, accompany frequent nose or gum bleeding, or occur with increasing warmth, swelling, pus, and pain.

Ask whether the marks are within the skin or on its surface, whether your medicines could affect bleeding, and whether examination elsewhere or blood testing is appropriate. Mention recent infections, injuries, travel, and any change in general health.
