What should you know about blood mucus in a cough?

Blood in mucus during a cough may reflect airway irritation or infection, but repeated bleeding needs medical assessment. A few streaks can follow hard coughing, a cold, or inflamed airways. The amount of blood, how long it has happened, and symptoms such as fever or breathlessness help determine urgency.

Mucus can become blood-streaked when the nose, throat, or breathing tubes are irritated. Acute bronchitis and a common cold can trigger frequent coughing; asthma can make airways sensitive, while bronchiectasis can produce repeated phlegm. Post-nasal mucus from allergic rhinitis or chronic cough can also irritate the throat. Acid reflux may contribute to a persistent cough. These conditions can present with the symptom, but the symptom alone does not identify which one is present.

Contact a clinician if the blood is new, returns, or continues. Record the colour, amount, timing, and whether it is mixed through mucus. Mention smoking or vaping, blood-thinning medicines, recent illness, allergies, reflux symptoms, and any known lung condition. Rest, drink fluids if suitable, and avoid smoke. Do not self-prescribe antibiotics or cough medicines without checking that they fit your symptoms and medicines.

Seek emergency help for ongoing or more-than-small-volume bleeding, severe difficulty breathing, chest pain, faintness, confusion, blue lips, or rapid deterioration. Prompt assessment is also needed when blood appears with a high fever, marked weakness, or anticoagulant use. These warning signs require attention independent of whether the cough began with a cold.
