What should you know about blood in cloudy urine?

Cloudiness can come from blood cells, white blood cells, crystals, mucus, semen, or concentrated urine. A urinary tract infection can present with cloudy urine, blood, burning, urgency, or an unusual smell. Kidney stones can present with blood and sharp pain that moves from the side towards the groin. A kidney infection can present with cloudy or bloody urine plus fever, chills, nausea, or back pain. Prostatitis can present with pelvic discomfort, painful urination, or difficulty passing urine. Prostate enlargement may affect flow in some men. Bladder cancer, prostate cancer, and sickle cell disease are other conditions that can present with blood in urine, but cloudy appearance cannot identify the diagnosis.

Book a clinical review and provide a clean urine sample if requested. Tell the clinician about the colour, smell, pain, fever, menstrual bleeding, recent exercise, sexual activity, and all medicines. Testing may include a dipstick, microscopy, and culture; the results help distinguish blood from infection or crystals. Do not use someone else’s antibiotics or delay review because the cloudiness improves. Keep drinking normally unless you have been told to restrict fluids.

Get urgent help if cloudy or bloody urine occurs with a high fever, shaking, severe flank pain, repeated vomiting, confusion, faintness, very little urine, or inability to urinate. Visible clots or rapidly increasing bleeding also warrant urgent assessment. Pregnancy and kidney disease lower the threshold for contacting a clinician.
