What should you know about blood in urine and throwing up?

These symptoms can occur together when the urinary system is irritated or obstructed, or when an infection causes pain and nausea. The appearance of blood may range from pink or red urine to tea-coloured urine, and blood may be visible or found only in a urine test.

Possible explanations include a urinary tract infection, a kidney infection, or kidney stones. A kidney infection may bring fever, chills, pain in the side or back, nausea, and vomiting. A stone can cause severe, wave-like side or groin pain and blood in urine; vomiting may follow the intensity of the pain.

Prostate inflammation can cause urinary symptoms and discomfort, while an enlarged prostate can affect urine flow. Less common but important causes include bladder or prostate cancer, and some inherited blood disorders such as sickle cell disease. Blood in urine does not identify one cause by itself, so testing matters.

Arrange a same-day medical assessment if you can see blood in your urine and are throwing up. A clinician may ask when the symptoms began, where pain is located, whether you have fever or burning when passing urine, and whether you can keep fluids down.

Take small, frequent sips of water or an oral rehydration drink if you can keep them down. Do not force fluids if vomiting is continuous. Note the urine colour, any clots, the amount and appearance of vomit, and medicines you take, including blood thinners. Do not start leftover antibiotics.

Go to an emergency department now if you cannot keep fluids down, feel faint or confused, have severe or worsening abdominal, side, or back pain, develop a high fever or shaking chills, pass clots, or cannot pass urine. Seek urgent help for very little urine, marked weakness, or vomit that contains blood or looks like coffee grounds.

Do not drive yourself if you feel faint, confused, or severely unwell. Bring a list of medicines and, if possible, a photo or description of the urine and vomit.

Ask what urine and blood tests are needed, whether imaging could check for a stone or blockage, and which symptoms should prompt immediate reassessment. Tell the clinician about previous urinary infections, stones, prostate problems, recent strenuous activity, injuries, and any chance of pregnancy.
