Blood in sputum when clearing your throat

Dryness, a nosebleed, gum irritation, post-nasal mucus or repeated throat clearing can leave a red streak on mucus. Cough-related airway inflammation can do the same. Acute bronchitis, pneumonia, bronchiectasis and COPD can present with blood in sputum, while chronic sinusitis can contribute to mucus in the throat. Blood noticed during throat clearing does not prove that the lungs are the source.

Describe whether you are coughing from the chest or clearing mucus from the throat, and note hoarseness, blocked nose, fever, chest pain, wheezing or breathlessness. This distinction helps a clinician decide whether to examine the upper airway, chest, or both.

Use gentle throat clearing, sip water and avoid smoke, vaping and other irritants. Check whether your nose or gums are bleeding, but do not probe or injure the area. Arrange medical advice if the blood returns, increases, follows a chest illness, or occurs with a persistent cough. Keep a record of episodes and medicines, including any that affect clotting.

Seek urgent help if you have trouble breathing, chest pain, faintness, confusion, blue lips, or bleeding that continues or is more than a small streak. A worsening cough with fever or marked weakness also warrants prompt assessment. Emergency symptoms take priority over trying to identify the source at home.
