What should you know about blood in sputum after waking up?

Mucus and a small amount of blood can collect overnight when you are lying down. Dry air, mouth breathing, congestion, or a minor nosebleed may let blood drain into the throat. A forceful first cough can then bring up this mixture. Repeated morning episodes can also occur with airway inflammation, bronchiectasis, COPD, or an infection such as acute bronchitis or pneumonia.

Look for clues before assuming the source: a dry or blocked nose, blood when blowing your nose, chesty cough, fever, wheezing, or breathlessness. The timing is useful information, but morning-only bleeding is not a diagnosis.

Keep a note of whether the blood is seen only in the first mucus of the day or continues later. Avoid smoking and smoky rooms, drink fluids, and try not to cough forcefully. If your nose feels dry, ask a pharmacist or clinician about a suitable saline product. Book a medical review if the episode repeats, the cough persists, or you have known lung disease. Do not use antibiotics without advice; the appropriate treatment depends on the cause.

Get emergency help for ongoing bleeding, more than a small amount of blood, breathlessness, chest pain, faintness, confusion, or a rapidly worsening illness. Contact a clinician promptly when morning blood keeps recurring, is accompanied by fever or night sweats, or occurs with a long-standing cough. Seek advice sooner if you take medicine that affects clotting or recently injured your chest.

Ask whether your nose and sinuses could explain the blood, whether your lungs need examination, and what symptoms should change the urgency. Mention smoking, recent infections, inhalers, blood-thinning medicines, and exactly when the blood appears.
