Blood in phlegm without a cough: what to know

A small streak may follow irritated gums, a nosebleed draining backwards, dry nasal passages, or a scratched throat. Acid reflux can irritate the throat, while a cold or allergic rhinitis can create mucus that carries a little blood. Blood that seems to come from deeper in the chest needs more careful assessment; bronchiectasis and other airway conditions can produce blood-stained phlegm, sometimes before a cough develops.

Spit the material into a tissue rather than swallowing it, and note its colour, amount, timing and possible source. Check gently for bleeding from the nose, gums or mouth. Drink fluids and avoid smoking or forceful throat-clearing. Arrange a primary-care appointment if it happens again, lasts beyond a brief episode, or you take blood-thinning medicine. A clinician may examine your mouth, nose and chest and decide whether further tests are needed.

Seek urgent medical help if bleeding is more than a few streaks, keeps returning over a short period, or comes with breathlessness, chest pain, faintness, a fast heartbeat, fever or marked weakness. Emergency care is appropriate if you are struggling to breathe or cannot stop bringing up blood. Do not drive yourself if you feel faint or severely short of breath.
