Blood in phlegm but no cough

Mucus can collect overnight and carry a small amount of blood from dry nasal tissue, irritated gums, a minor mouth injury, or post-nasal drainage. Allergic Rhinitis and a Common Cold can inflame the nose and throat. Acid Reflux can irritate the throat and leave a blood-streaked sensation or taste. Acute Bronchitis, Asthma, Bronchiectasis, or Chronic Cough are more relevant when there is coughing, wheezing, breathlessness, or chest discomfort; the absence of a cough makes the source less clear rather than proving it is harmless.

Check gently for bleeding around the gums, tongue, cheeks, and nostrils. Notice whether the material is clear mucus, a clot, bright red blood, or dark residue, and whether it occurs after sleep, brushing, meals, or nose blowing. Drink water, avoid smoking and vaping, and do not pick or forcefully blow your nose. Arrange an appointment if the finding returns, persists, or cannot be traced to a visible minor source.

Seek urgent care if the amount is more than a small streak, bleeding continues, or you develop breathlessness, chest pain, faintness, confusion, or marked weakness. Emergency help is appropriate if blood is difficult to clear from your mouth or breathing is affected. Tell the clinician about fever, weight change, medicines that affect clotting, smoking, and any recent dental or nasal procedure.
