Blood in mucus when coughing: possible causes

Bright red streaks can appear after forceful coughing because delicate airway lining becomes irritated. Blood may also drain from a dry or inflamed nose into the throat. Conditions such as acute bronchitis, a common cold, asthma, bronchiectasis, or a chronic cough can produce mucus and coughing; acid reflux and allergic rhinitis may irritate the upper airway and contribute to throat clearing. The pattern matters: note whether the blood is mixed through phlegm or sits on its surface, whether it follows a nosebleed, and whether fever, breathlessness, chest pain, wheezing, or weight change is present.

Rest your voice and avoid smoke, vaping, and dusty air. Sip fluids if swallowing is comfortable, and use a humidified room if dry air triggers coughing. Do not start antibiotics or stop a prescribed blood-thinning medicine on your own. Arrange a clinical review if the blood returns, lasts beyond a brief episode, or occurs with a persistent cough. A clinician may examine your nose, throat, and chest and decide whether further tests are needed.

Seek emergency help for more than a small streak of blood, bleeding that does not settle, difficulty breathing, blue lips, faintness, severe chest pain, or confusion. Get same-day advice if you have a high fever, worsening breathlessness, feel unusually unwell, or take anticoagulant medicine. Do not drive yourself if you feel faint or struggle to breathe.
