Blood in cough from the nose and throat: what to know

Small vessels in the nose and throat can bleed after dryness, vigorous blowing, repeated coughing, or inflammation. A nosebleed may run backward when you lie down, leaving blood in saliva or mucus the next time you cough. Common Cold and Allergic Rhinitis can cause congestion and postnasal drainage; Acid Reflux can irritate the throat and encourage throat clearing.

A clinician may still need to consider the lower airways. Acute Bronchitis, Asthma, Bronchiectasis, and Chronic Cough can change sputum and cause coughing. The appearance alone cannot reliably locate the source.

Sit upright and gently spit out mucus rather than swallowing it. If the nose is bleeding, use gentle pressure and avoid repeated checking or forceful blowing. Keep a note of the amount, colour, timing, and whether blood is present without coughing. Seek medical advice when episodes recur, the cough persists, or you develop fever, wheeze, chest discomfort, or unusual tiredness. Do not alter prescribed medicines without advice.

Call emergency services for heavy or continuous bleeding, breathlessness, chest pain, faintness, confusion, blue lips, or an inability to clear the airway. Prompt assessment is appropriate if you use blood-thinning medicine, have significant lung disease, or cannot distinguish a nose or throat source from a lung source.

Ask for help locating the source and for advice on nasal care, reflux management, or cough treatment suited to your situation. Mention allergies, recent respiratory infection, smoking or vaping, asthma, and any history of Bronchiectasis.
