Blood During Sex but No Period

A small smear can follow friction or dryness, but bleeding may instead start at the cervix or inside the womb. Missing periods and bleeding with sex are separate clues that need to be considered together rather than treated as proof of one cause.

Timing provides useful clues. Blood seen only after penetration can follow a small vaginal injury, dryness, cervical inflammation, infection, or a cervical polyp. Bleeding that also occurs at other times may come from the womb. Some cervical, vaginal, and womb disorders, including cancer, present with unexpected bleeding, although the symptom does not show which diagnosis is present.

Periods may stop because of pregnancy, menopause, polycystic ovary syndrome, premature ovarian insufficiency, hormonal contraception, physical stress, or another endocrine problem. Turner syndrome can be associated with ovarian insufficiency and absent periods. A medical history and examination help determine whether these issues are connected.

Stop if sex is painful and avoid further penetration until active bleeding has settled. Write down the dates, whether blood appeared during or after sex, how much there was, and whether you noticed dryness, discharge, odour, pelvic discomfort, urinary symptoms, or bleeding at other times. Water-based lubricant may reduce friction but should not delay assessment of recurring blood loss.

Use a home pregnancy test when conception is possible. Make a routine appointment after a first unexplained episode and seek an earlier review if it returns. Assessment can include viewing the cervix and vaginal tissue, checking for infection, and selecting further tests based on age, pregnancy possibility, medicines, and menstrual history.

Attend urgent care if you are soaking pads with blood, feel faint or confused, have severe pelvic or abdominal pain, develop shoulder-tip pain, or feel feverish and unwell. If pregnancy could be possible, pain with bleeding needs rapid evaluation for complications such as ectopic pregnancy.

Do not wait for another episode if bleeding begins after menopause. Persistent postcoital bleeding, a new offensive discharge, or pain that is getting worse also calls for timely clinical review.

Ask whether the examination shows dryness, injury, infection, or a change on the cervix; whether pregnancy and infection testing are appropriate; and whether cervical screening needs attention. Mention the date of your last period, contraceptive or hormone use, previous cervical results, and every medicine or supplement you take.
