What should you know about blood at the end of vomiting?

Small streaks may occur when repeated retching irritates the throat or upper digestive tract. Fresh red blood, dark material resembling coffee grounds, or blood mixed with vomit can indicate bleeding higher in the digestive system. Causes of vomiting such as food poisoning, gastroenteritis, migraine, or rotavirus infection do not explain every episode of bleeding. Other conditions, including gallstones or pancreatitis, may cause severe abdominal symptoms and require a clinician’s assessment. Meningitis is a separate emergency concern when vomiting occurs with a severe headache, stiff neck, confusion, or sensitivity to light.

Stop alcohol and avoid medicines such as aspirin or ibuprofen unless a clinician has specifically advised them. Do not eat a large meal while actively vomiting. If you can swallow safely, take small sips of fluid, and note the colour and approximate amount of blood. Arrange same-day medical advice even if the bleeding stops, particularly if you take a blood thinner, have liver disease, or have had previous digestive bleeding.

Call emergency services for ongoing or heavy bleeding, faintness, confusion, clammy skin, fast breathing, severe abdominal pain, black stools, or inability to keep fluids down. Do not drive yourself if you feel faint. A child, older adult, or pregnant person with blood in vomit should receive urgent clinical guidance.

Tell the clinician when the bleeding started, whether it followed repeated retching, what it looked like, and whether you have pain, black stools, fever, or dizziness. Ask which medicines to avoid and whether examination or testing is needed.
