Bleeding after sex following a hysterectomy

Soon after surgery, penetration can disturb healing tissue. Long after recovery, dryness, friction, inflammation, a small area of fragile tissue at the top of the vagina or another vaginal or cervical problem may be responsible. The cause cannot be identified from the colour alone.

The type of hysterectomy matters. If the cervix remains, bleeding can come from the cervix; if it was removed, blood may arise from the vaginal walls or the healed surgical join called the vaginal cuff. Dry or delicate vaginal tissue can split with friction, particularly around or after menopause. Infection, a polyp or abnormal tissue also needs consideration.

Post-sex bleeding is a symptom with several explanations. It does not by itself establish cervical cancer, womb cancer or any other condition, but examination is worthwhile when it persists.

Pause penetrative sex until the bleeding has stopped and the area has been assessed if episodes recur. Record the amount, colour, timing, pain, discharge and whether bleeding happens only after contact. A pad can help you monitor it. Bring the date and type of hysterectomy if known, including whether the cervix was retained and the reason for surgery.

A clinician may inspect the vulva, vagina, vaginal cuff and remaining cervix when relevant. Swabs or other tests depend on what the examination finds.

Seek urgent care for heavy or ongoing bleeding, large clots, faintness, breathing difficulty, severe pelvic or abdominal pain, fever, foul-smelling discharge, or a feeling that tissue is bulging from the vagina. After recent surgery, contact the surgical team promptly about fresh bleeding or worsening pain.

Arrange a non-emergency gynaecology visit for any unexplained episode that repeats, even if each episode is light and stops quickly.

You may want to ask whether your cervix was removed, whether the vaginal cuff has healed, whether dryness or infection is visible, which tests are appropriate and when sexual activity is safe to resume. Also describe medicines that affect bleeding and any previous abnormal cervical results.
