Bloating and pain during sex

The location and timing of pain provide useful clues. Pain at the vaginal entrance may relate to dryness, irritation, a skin condition, or pelvic-floor tension, while deeper pain can occur with pelvic inflammation, endometriosis, an ovarian problem, or pressure from the bowel or bladder.

Record whether pain is burning, sharp, aching, or pressure-like; whether it occurs on entry or with deeper penetration; and whether bloating is linked to meals, bowel movements, or the menstrual cycle. Discharge, bleeding after sex, painful urination, constipation, fever, and changes in periods can narrow the possibilities.

Endometriosis can present with deep pain during sex and pelvic or digestive symptoms. Genitourinary syndrome of menopause can present with vaginal dryness, burning, and painful sex. Pelvic inflammatory disease may present with pelvic pain, unusual discharge, bleeding, or fever. Symptoms overlap, so none of these patterns confirms a diagnosis by itself.

Pause any activity that hurts; pain does not need to be pushed through. If dryness or friction contributes, a suitable lubricant and slower, shallower penetration may reduce irritation. Avoid perfumed washes or internal cleansing if the vulva or vagina feels sensitive.

Book a clinical assessment if the combination recurs, persists, or changes your sexual activity. Depending on the history, evaluation may include an abdominal or pelvic examination, infection testing, pregnancy testing, or imaging. You can request a chaperone and ask the clinician to stop an examination at any point.

Seek urgent medical advice for pelvic pain with fever, vomiting, unusual or foul-smelling discharge, difficulty passing urine or stool, blood in urine or stool, or if you are pregnant or might be pregnant. Persistent bloating, early fullness, reduced appetite, unexplained weight loss, or bleeding after menopause also needs medical review.

Go to emergency care for sudden severe or escalating pelvic pain, fainting or marked dizziness, shoulder-tip pain, heavy vaginal bleeding, breathing difficulty, or pain with a rigid or very tender abdomen.

Ask which causes fit the location of your pain, whether infection testing or imaging is appropriate, and whether pelvic-floor assessment could help. Share your menstrual pattern, contraception, pregnancy possibility, bowel and bladder changes, vaginal symptoms, previous pelvic surgery, and what makes the pain better or worse.
