Skin blister causes and clues

A blister is a pocket of fluid created when skin layers separate. Its timing and pattern matter more than the fluid alone: a single rubbed spot differs from a spreading, painful, itchy, or widespread eruption.

Friction and minor burns tend to match a clear site of contact. Contact dermatitis can present with itchy blisters, redness, and scaling where an irritant or allergen reached the skin. Dyshidrotic eczema can present with intensely itchy small blisters on the hands or feet.

Viral illnesses may produce multiple lesions. Chickenpox can present with widespread itchy spots that become blisters, and shingles can present with pain or tingling followed by grouped blisters in a limited band. A clinician considers recent medicines when blistering is extensive or involves moist linings such as the mouth.

Note when the eruption started, whether new blisters are appearing, and what contacted the skin beforehand. Photographs taken as the pattern evolves can help at an appointment. Include new footwear, adhesives, plants, chemicals, cosmetics, medicines, fever, pain, and sick contacts.

Meanwhile, remove an obvious irritant, cool a minor heat-exposed area with cool running water, and protect intact blisters from pressure. Use gentle cleansing and a non-stick dressing if skin has opened. Avoid popping lesions or applying a strong medicated cream before the cause is understood.

Arrange prompt care if blisters are rapidly multiplying, very painful, infected-looking, associated with fever, or located near an eye. Mouth, lip, genital, or widespread blisters require medical assessment, particularly after starting a medicine.

Call emergency services for breathing difficulty, swelling of the face or tongue, collapse, extensive peeling, or inability to swallow fluids. A chemical or electrical burn also needs urgent professional guidance rather than routine home blister care.

Ask whether the distribution suggests an external trigger, eczema, or infection; whether testing is needed; what you should avoid; and whether close contacts need precautions. Bring a list of medicines and supplements, including recently stopped items, so timing can be checked carefully.
