Blighted ovum miscarriage: what to expect

You may have bleeding and cramping, but some people have no clear symptoms before an ultrasound. The experience and recovery differ, so your clinician will confirm the diagnosis and discuss which approach fits your health and preferences.

An anembryonic pregnancy, often called a blighted ovum, happens when the gestational sac forms but an embryo does not develop. Pregnancy hormones can remain present for a while, so nausea, breast tenderness, or a positive pregnancy test may continue even though the pregnancy cannot progress.

Diagnosis is based on ultrasound findings interpreted in relation to the pregnancy dates and, when needed, repeat assessment. A transvaginal ultrasound gives a close view of the uterus. This loss is not caused by ordinary exercise, sex, work, or something you ate.

Your options may include waiting for the pregnancy tissue to pass naturally, taking medicine that helps the uterus empty, or having a procedure to remove remaining tissue. Waiting can involve uncertain timing. Medicine commonly brings cramping and bleeding at home. A procedure is more predictable but involves an intervention and its associated risks.

Use pads rather than relying on appearance alone to monitor bleeding, take only pain relief recommended for you, and arrange the follow-up your care team specifies. Follow-up may include symptom review, pregnancy-hormone testing, or ultrasound to check that the uterus has emptied. Emotional reactions can range from sadness to numbness; support from someone you trust or a counsellor may help.

Seek urgent care for bleeding that rapidly soaks pads, fainting, severe or worsening abdominal pain, shoulder-tip pain, trouble breathing, confusion, or marked weakness. These symptoms require prompt assessment rather than waiting for a routine appointment.

Contact your maternity or gynaecology team promptly for fever, chills, unpleasant-smelling vaginal discharge, pain that is not controlled by the advised medicine, or bleeding that becomes heavier instead of settling. These can signal infection, significant blood loss, or tissue remaining in the uterus.

Ask how the diagnosis was confirmed, whether another scan is appropriate, what each management option would involve, which symptoms should trigger urgent care, and how follow-up will show that the miscarriage is complete. You can also ask when to resume exercise, sex, tampons, travel, or attempts to conceive based on your recovery.
